Eligibility Requirements:

To receive services, individuals must meet the following criteria:

  1. Be 18 years of age or older.
  2. Have a documented mental health or behavioral condition, as determined by a diagnostic assessment.
  3. Have significant challenges in multiple areas of functioning (e.g., social, employment, independent living) as indicated by a functional assessment.
  4. Demonstrate the need for support across these key life areas, as determined by the assessment.
  5. The individual must be able to provide informed consent for care or have a legal guardian or representative provide consent on their behalf.
  6. Individuals with a history of violent or criminal behavior that could jeopardize the safety of others or themselves may not be eligible for certain services.
